Customer Reference / Satisfaction Form

Hi All,

Has anyone got a blank form for customers references that i can use as a template for my own. Is there one somewhere on this site ?
Have decided that the use of photos and references from previous customers may help get more work, but not sure of the form that i should use and what type of info should be on the form.

Hope someone can help me out on this one !!!

it's a bit hit and miss mate,you could make them all up as you go to price jobs(not implying anything) would it not be an idea to get the customers name and number and write it on the back of your photos and ask them if it would be ok for future clients to contact them,maybe even offer the customer a discount the next time they use you as a reward for the reference

why dont you get a portfolio made up and show customers your work and have references at hand.
