I prefer fischer cavity anchors myself over toggle bolts. If i'm using these then I insert them into the wall before tiling then place a precut tile over the top

This is a rad I did this week with them, three fixings are more than enough for the weight of this radiator

Thats the ones i was talking about, but as you say have to go in first!
 
why do they have to go in first?? and what do you do with them spiky bits under the head?

They have to grab into the plaster and cant grab the tiles so they just spin if you try them after.
 
and if you use plugs of any sort , make sure you knock them past the tiles , you dont want anything expanding where the tile is as you screw the screw in to tighten :incazzato:

If you use this type, invest in the 'gun' for squeezing' them if you rely on the screw to squeeze them the 'spikey' bits can drill through your plaster board
Plasplugs plasterboard plugs would be my choice this time round and as Paul said, clearance hole in the tile
 
Push rawl plugs deeper than the tile so that the weight is not on the tile (as mentioned above by Paul)

For things like bolts, loo-pans, sinks, in fact anything heavy duty
at sizes 10mm 12mm 14mm then open a hole as wide as you can in the tile

14mm bolt.

Remember by taking plugs out of tiles that there is clearance for your bolts, and you do get some degree of error correction by movement. Of course the fitting will
conceal the final hole size (just dont go too big !! )
