I thought about centre line to centre tile but it doesn't look right when you see it like that.

I prefer it the way Simons70 done it as well and with no other directive that's how I would have done it.

I prefer the way you did it. But customers etc, dont realise when setting out you have to look around the whole room and sometimes having tiles centering on a certain item may not work.

The site manager want to start with a small cut end in the center of toilet,not with full tile like this.

tell the site agent that you'll re-do it the way he wants it,,,as an extra. See if he sticks to his guns then

I agree, just take them down (6 tiles no big deal) and put them where they want.
