When you guys overboard the subfloor with a backer board, do you undercut the door frame in order to fit the backer board and tile/adhesive under?

also what do you use to hide the height difference between the tiled floor and whatever is in the adjacent room ( wood floor in my case)
Im anticipating a fairly big step (6mm backer +10mm tile + adhesive)

Had a look at different threshold covers but don't seem to be able to find any that cover large differences in height.
are there any other products that i can use to neatly hide the expansion gap and step
